Long-Term National D Visa

Documents Required

Application Form

• Fully completed and signed.

Travel Document

• A passport that is:
  • Not older than 10 years
  • Contains at least two blank pages
  • Valid for at least three months beyond the intended visa duration

Proof of Accommodation

One of the following must be provided:

• Ownership certificate of an apartment or house
• Lease/sublease agreement
• Accommodation confirmation from a legal entity

Proof of Finance

• For non-business purposes:
  • Must show 15 times the subsistence minimum plus 2 times the minimum per additional month.
  • As of 2023, the subsistence minimum is 3,130 CZK/month.
  • Example: A six-month stay requires 78,250 CZK.
• For business purposes:
  • Must show 50 times the subsistence minimum (156,500 CZK).

Accepted proofs of finance:

• Bank statement showing sufficient funds
• A valid internationally recognised payment card
• A scholarship confirmation or sponsorship letter (for students)

Photograph

• Recent passport-sized colour photo

Proof of Purpose of Residence

Depending on the visa purpose, one of the following:

• Employment contract
• University/school admission letter
• Business registration certificate
• Scientific research agreement
• Family reunification documents (e.g., marriage/birth certificate)

Criminal Record Certificate

• Police clearance certificate from the applicant’s home country.
• Additional certificates may be required for applicants who have lived in other countries for more than six months in the last three years.

Parental Consent (For Minors)

• A written consent letter from both parents or legal guardians (if they are not accompanying the minor).

Medical Report (If Requested)

• Some applicants may be required to provide a medical certificate.

Photo Specifications

• Recent, colour passport-sized photograph.
• White or light background, no shadows.
• Neutral expression, no smiling.
• No sunglasses, hats, or head coverings, except for religious purposes.
• Must be glued to the visa application form.

If the submitted photograph does not meet these requirements, the visa application will be considered incomplete, and the applicant will be required to submit a new photo.
